Your support coordinator will meet with you one-on-one to explore your goals and needs.
Together, you'll create a tailored plan for your short and long-term goals, which will help guide and motivate you moving forwards to achieve them.
We'll work with you to identify suitable supports who will be the best fit for you and your life.
We'll regularly check in with you to make sure your NDIS plan reflects your evolving needs and goals and support you with any plan reviews.
You'll gain the understanding, knowledge and skills over your NDIS plan, budgets and support options to have greater confidence and control in your life.
A support coordinator helps you understand your budgets and support categories, and works with you to find and organise the best supports for your needs. It’s what the NDIS calls a ‘capacity building’ support – designed to help you build the skills and confidence to manage your own supports in future.
Coordinating your supports can be a little daunting and time-consuming. At RenaissCare, you get your own support coordinator who has a deep understanding of the service providers and legislations within your state. Support coordination is especially useful if you need to engage lots of different service providers.
The NDIS looks at each participants need on a case-by-case basis to assess whether support cordination is required. During your planning meeting with the NDIS you can request funding for support coordination who in turn looks at your personal situation for support cordination inclusion.
